We are seeking compassionate and skilled Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to join our innovative care team. Job Responsibilities

Conduct comprehensive patient assessments, including annual wellness exams, virtually and in patient homes.
Educate patients and families on health benefits, programs, and preventive care measures.
Develop and implement individualized care plans, ensuring optimal patient outcomes.
Collaborate with primary care providers, specialists, and care team members to coordinate care.
Prescribe medications, order diagnostic tests, and monitor patient progress.
Accurately document patient information in EMR systems (e.g., EPIC).
Participate in integrated care team meetings to optimize care delivery.

Qualifications

Licensure: Active and unrestricted Nurse Practitioner or Physician Assistant license in the state of practice.
Certification: Board-certified (NP: ANCC/AANP; PA: NCCPA).
Experience: At least 2 years of direct patient care experience, preferably in chronic disease management or managed care.
DEA Certification: Current and unrestricted.
Technical Proficiency: Familiarity with EMR systems and remote care tools.
Preferred: Bilingual (English/Spanish) and experience conducting wellness visits.

Working Hours

Monday - Friday: 6-8 Visits per day
New Patient visits 90 mins
Member visits 15-60 minutes
No more than an hour drive between visits

Patient-Centered Care: Demonstrated ability to deliver compassionate, personalized care for patients with chronic conditions, focusing on improving quality of life.
Clinical Expertise: Proficient in conducting comprehensive assessments, prescribing medications, and developing evidence-based treatment plans.
Team Collaboration: Skilled in working within an integrated care team, including RNs, social workers, and physicians, to coordinate and optimize patient care.
Technology Proficiency: Experienced with electronic medical records (EMR) systems, such as EPIC, and remote patient monitoring tools.
Effective Communication: Strong verbal and written communication skills for patient education, interdisciplinary collaboration, and documentation.
