Job Description
We are seeking skilled Locum Advanced Practice Providers (Nurse Practitioners or Physician Assistants) for a minimum 90-day assignment. This role is ideal for providers eager to make a meaningful impact in patients' lives while working as part of an integrated care team.
In this position, you will deliver personalized, compassionate care to patients managing chronic conditions such as CKD (Chronic Kidney Disease) and ESRD (End-Stage Renal Disease). You’ll play a key role in improving patient quality of life by addressing medical needs, reducing avoidable hospitalizations, and fostering better health outcomes.
Job Responsibilities
• Educate patients and families on health management and preventive care.
• Document medical histories, assessments, and care plans accurately.
• Coordinate care with primary care providers, specialists, and ancillary services.
• Prescribe medications, order diagnostic tests, and collaborate with supervising physicians.
• Actively participate in integrated care team meetings to optimize patient outcomes.
Qualifications
• Certification: Board-certified (NP: ANCC/AANP; PA: NCCPA).
• Experience: At least 2 years of direct patient care, preferably in managed care or chronic disease management.
• DEA Certification: Current and unrestricted.
• Other: Reliable transportation, valid driver’s license, and car insurance.
Working Hours
Credentialing take roughly 30-60 days to complete. In the interim, the APPs can function under their RN license until cleared.
In person/In home assignment - our APPs go to the patients' homes directly. Usually 4-6 visits per day.

Expected to travel approx. 1 hour radius in any direction from the targeted city may have a slightly larger area.
Skills
Experience in Managed Care:
Previous experience with health plans, IPAs, or care management programs.
Preventive Care Expertise:
Experience conducting wellness visits and closing care gaps.
