Our client, a home health agency in Colorado, is seeking a locum Family Practice Nurse Practitioner for a 60-day assignment starting in February 2025, with potential for extension. The role offers a flexible schedule between 8:00 AM and 8:00 PM, with a minimum requirement of 8:00 AM to 5:00 PM, and a preference for full 12-hour days. The NP will conduct Medicare assessments, in-home patient evaluations, and required screenings such as Dexa, Retinal Screening, and FOTB. Responsibilities include reviewing medical histories, medications, and completing HRA/Charts. No stool or blood collection or procedures are required. The position involves traveling to patients' homes, with a preferred radius of 30 miles or greater. The agency uses Focus Care EMR system.

Job Overview:

Job Details:

Responsibilities include:

- Conducting Medicare Assessments

- Reviewing medical history and medications

- Completing HRA/Charts

- Performing in-home patient assessments

- No stool or blood collection required

- No procedures needed

- Training provided for required screenings

- Opportunity for additional assessments based on provider availability

- Travel radius determined by the provider (larger radius means more hours)


Requirements:

- Licensed Nurse Practitioner with Family Practice specialization

- Active Colorado NP license or ability to obtain one

- Willingness to travel within a 30-mile radius or greater

- Comfort with in-home patient assessments and screenings

- Proficiency in Focus Care EMR system
