Optima Health is seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality care in Skilled Nursing Facilities (SNF) and Assisted Living Facilities (ALF) in Riverside, California.

Job Responsibilities

Provide comprehensive patient care in SNF and ALF settings.
Conduct assessments, diagnose conditions, develop treatment plans, and manage ongoing care.
Collaborate with facility staff, physicians, and other healthcare professionals to ensure optimal patient outcomes.
Maintain accurate and up-to-date patient records.
Participate in care coordination and case management to enhance patient quality of life.

Qualifications

Valid and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of California.
Experience working in SNF or ALF settings preferred but not required.
Strong communication and interpersonal skills.
Ability to work independently and manage a flexible schedule.
Must have a valid DEA license.
