Primary Function:

  • Developing Role: Source potential admissions for inpatient care at home (from ED or inpatient units) and complete the admission process with a virtual attending physician. You ll be onsite at the hospital, supported by a robust team of APPs and physicians. Participate in daily huddles for collaborative patient care.
  • Evolving Responsibilities: Currently, attending physicians handle daily rounds, but as the program grows, APPs will have opportunities for daily rounding. This may include virtual and in-person visits, supported by paramedics and virtual RNs.

What to Expect:

  • Growth Mindset: Participate in program design and iteration.
  • Swing Shift Hours: Work 12pm-10pm, with flexibility to rotate into day shifts (7am 7pm), no night call currently, possible in the future. Some weekends, evenings, and holidays required. APPs participate in a self-scheduling process that allows for flexibility.
  • Locations: Team members are expected to rotate through our three Treasure Valley hospital sites (Boise, Meridian, Nampa) and are typically assigned one site during their shift with potential to cover more than one site as needed for staffing shortages. The primary site for swing shift will likely be the Meridian Hospital but is still in process of being determined. Patient home visits are not currently done by the team members in this role, but willingness to travel to a home is required should this role develop to include that function.

Key Clinical Skills:

  • Patient evaluation, acute care assessment, defining an assessment and plan, and completing documentation in electronic medical record.
  • Prefer experience with caring for acutely ill patients and admitting them to hospitals.
  • Effective communication with patients and families.
  • Collaboration with interdisciplinary teams.
  • Adaptability: This is a groundbreaking program where medical-surgical care is provided in the home. This is the first of its kind in Idaho. Being inspired to help build this program and adapt as it changes is critical.
