We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our outreach team in Hemet, CA. This position involves providing high-quality care at various outreach facilities, including local schools, to improve community health outcomes. The ideal candidate will have a passion for community-based care and the flexibility to travel to different locations.
Key Responsibilities:
- Provide direct patient care, including assessment, diagnosis, and treatment for individuals and families at outreach facilities.
- Conduct school-based health services, promoting wellness among children, adolescents, and staff.
- Collaborate with an interdisciplinary healthcare team to ensure a patient-centered approach.
- Participate in case management meetings, staff meetings, and community health initiatives.
- Deliver health education and preventive care classes tailored to community and school needs.
- Complete and maintain all required documentation, including billing, patient forms, and medical records.
- Supervise clinic support staff, including Medical Assistants, and other team members as needed.
- Ensure compliance with all safety protocols, policies, and regulatory guidelines.
- Participate in after-hours or weekend on-call schedules, as directed by the Medical Director.
Qualifications:
- Graduation from an accredited Nurse Practitioner program.
- Current Nurse Practitioner license in California.
- Valid California driver’s license and reliable transportation for traveling between outreach locations.
- Ability to pass a pre-employment physical examination.
- Experience in community or school-based care (preferred).
- Bilingual skills (preferred).
