We are in search of a Perm/Full-Time Geriatric NP for North Central Indiana. The region is located East of South Bend, Indiana.

The ideal Provider will have at least 1 year of experience working with Geriatric Populations.

Location: North Central Indiana
Job Type: Perm / Full-Time
Shifts: full time, 8a-4p M-F + rotating call one week on, one week off

Practice Details:

• Care and services include: Adult day health services such as recreational and social programs Primary and specialty medical care Nursing care Prescription and over-the-counter medications Physical, occupational and speech/language therapies Social work services Vision and hearing care Dental care Personal care services Spiritual care and support Transportation Hot meals

• A PACE program, (Program of All-inclusive Care for the Elderly). PACE programs are centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible
• Full spectrum of health care services, from primary to acute to long-term care for frail elderly individuals who are certified to require nursing home care.
• Clinical and rehabilitative services are coordinated and participants visit, socialize and build meaningful relationships with doctors, nurses and other program participants.
• Provide the entire continuum of care and services to seniors with chronic care needs while maintaining their independence in their homes for as long as possible.

Requirements:

• Active IN License
• BLS, CDS, DEA
• Board Certified by ANCC or AANP.
• A minimum of one (1) year working with the frail or elderly population strongly preferred
