Priority Care Nurse Practitioner - Primary Care - Transitional Care

Our client, an established and growing provider organization, needs an experienced Nurse Practitioner to provide post-discharge clinical care for a select patient population.

We are seeking a Nurse Practitioner for a Priority Care Program. In this role, you will join an innovative care delivery team located in the San Fernando Valley.

You will be an integral part of a health care delivery program designed to reduce readmission rates for patients with comorbid conditions.

Position Details:

  • Provides comprehensive care in an outpatient clinic or community environment.
  • Emphasis on post-discharge patient care leading to sustained stabilization
  • Member of a supportive, collaborative Care Coordination team
  • Ensures clinical guidelines are met to ensure quality of care
  • Provides follow-up and referrals to social and behavioral health services
  • Ensures the provision of high-quality, transitional care for more complex patients
  • Facilitates interdisciplinary meetings for coordination of care
  • Monday - Friday, 8:00 a.m. to 5 p.m.

Qualifications:

  • Current California Nurse Practitioner License
  • DEA Certification
  • Current BLS Certification
  • Ability to work independently and direct care coordination
  • 3-4 years NP experience
  • Skilled in the management of CHF/COPD patients
  • Prior expertise providing care to complex patient populations
  • Experience in a home care setting is a plus

Compensation:

  • $140,000 to $160,000 per year, depending on experience
  • Sign-On Bonus
