We are seeking a dedicated and compassionate Family Medicine Nurse Practitioner to join our team. This hybrid role offers a balance of in-person and remote work, providing flexibility while ensuring high-quality patient care. The ideal candidate will have a strong background in family medicine and a commitment to patient-centered care.

Job Responsibilities

Conduct comprehensive patient assessments, including history taking, physical examinations, and diagnostic testing.
Develop and implement individualized treatment plans for patients of all ages.
Provide preventive care, health education, and counseling to patients and their families.
Manage chronic conditions and acute illnesses, coordinating care with other healthcare providers as needed.
Perform minor procedures and assist with more complex procedures as required.
Utilize electronic health records (EHR) to document patient encounters and ensure continuity of care.
Participate in quality improvement initiatives and contribute to the development of clinical protocols.
Maintain up-to-date knowledge of best practices and advancements in family medicine.

Qualifications

Master's degree in Nursing from an accredited program.
Current Nurse Practitioner license in MD.
Certification as a Family Nurse Practitioner (FNP) by a recognized certifying body.
Minimum of 5 years of experience in family medicine or primary care.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Strong organizational and time management skills.

Working Hours

Full Time
Monday - Friday
8am - 5pm
3-4 days onsite
1-2 remote
