SSM Health is seeking a compassionate and dedicated Family Nurse Practitioner (FNP) to join our healthcare team in St. Peters, Missouri. The FNP will provide high-quality primary care services to patients of all ages, emphasizing health promotion, disease prevention, and patient education. The ideal candidate will be committed to delivering patient-centered care and improving the overall health of the community.
Key Responsibilities:
- Conduct comprehensive health assessments, including patient histories, physical exams, and diagnostic tests.
- Develop and implement individualized care plans based on patient assessments.
- Diagnose and manage acute and chronic illnesses.
- Prescribe medications and other treatments in accordance with state regulations and practice guidelines.
- Provide preventive care, including routine check-ups, immunizations, and health education.
- Collaborate with physicians, specialists, and other healthcare professionals to ensure continuity of care.
- Educate patients and families on managing medical conditions and promoting healthy lifestyles.
- Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
- Participate in quality improvement initiatives and professional development activities.
Qualifications:
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
- Current licensure as a Nurse Practitioner in the state of Missouri.
- Certification as a Family Nurse Practitioner (FNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
- Minimum of 2 years of experience in a primary care setting preferred.
- Excellent clinical, diagnostic, and therapeutic skills.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team-oriented environment.
- Proficiency with electronic health records (EHR) systems.
Work Environment:
- Full-time position with regular office hours.
- May require occasional evening or weekend shifts based on patient needs.
