Duties and responsibilities include but are not limited to:

  • Performing initial patient screenings, patient history, physical examinations, and preventive health measures.
  • Demonstrating proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation of chronic patients required
  • Providing in-patient and virtual, video visits.
  • Ordering and interpreting health assessments and recommending appropriate actions.
  • Prescribing and reviewing medication orders.
  • Coordinating follow-up care and collaborating with the Clinic Physician, referring physicians, health providers, and hospital staff, when appropriate.
  • Participating in quality improvement activities and developing strategies that help improve patient care delivery, and cost-effectiveness
  • Safeguarding patient and Company data.

REQUIREMENTS:

If Nurse Practitioner:

  • Active RN and APRN certifications and licenses, including prescriptive authority, in California
  • Active board certification as a NP in California

If Physician Assistant:

  • Active PA board certification in California
  • Prescriptive authority

All Nurse Practitioner and Physician Assistant applicants must have:

  • Either a NP or PA license, including prescriptive authority, in state(s) of practice
  • Active DEA license or ability to apply for such license prior to or upon hire
  • Current CPR and BLS
  • Excellent verbal and written communication, computer, and interpersonal skills to effectively interact with a diverse inter-professional team and patient population.
  • Intermediate computer skills and at ease with learning and adapting to new technologies
  • Some experience as primary care provider in a clinical setting is preferred but new grads are welcome.
  • Proven skills, knowledge base, and judgment necessary for independent clinical decision-making
  • Adaptable, organized, strategic problem-solver, and a leader
  • Passion for improving community health
