Background Desired - Physician Assistant or Nurse Practitioner
- Family Nurse Practitioner / FNP, APRN, Adult-Gerontology Nurse Practitioner, AGPCNP, Physician Assistant, PA-C
- Geriatric, SNF, Hospice, or Palliative Care experience preferred
- 1+ years of experience as an Advanced Practice Provider
