Responsibilities:
The Advanced Practice Provider (Nurse Practitioner/Physician Assistant) within the ambulatory
setting works in collaboration with, and is an integral member of, a multidisciplinary health care
team. The PRIMARY CARE APP provides comprehensive care in a FAMILY MEDICINE or
INTERNAL MEDICINE practice.
Overview:
• Provides primary care to patients across the life span (family medicine) or adult patients
(internal medicine).
Assessment/Diagnosis and Management of Patients in Primary Care Setting
• Provides preventive health, including well visits, screenings, and immunizations; as well
as acute and complex care visits requiring diagnosis, treatment/management, and referral
to specialists or higher level of care when necessary.
• Collaborates with interdisciplinary health care team.
• Possesses awareness and provides assessment of patient risk factors, including physical,
socioeconomic, and psychosocial risk factors.
• Obtains and documents thorough patient history.
• Performs and documents comprehensive physical exam.
• Orders, performs, and interprets appropriate diagnostic testing, and responds to results
requiring further evaluation or investigation.
• Formulates differential diagnoses by priority.
• Engages/Collaborates with collaborative physicians when needed (i.e. diagnosis in
question, patient not responding to treatment, etc.).
• Coordinates with other specialists as needed.
Formulates Plan of Care
• Identifies expected outcomes from diagnosis, and formulates and documents a plan of
care to address preventive care, acute care, and chronic health care needs.
• Utilizes evidenced-based practice guidelines and protocols in an individualized, dynamic
plan of care that can be applied across the continuum.
• Implements and modifies plan of care based on patient response, diagnostic testing, and
specialist recommendations.
• Prescribes diagnostic strategies and therapeutic interventions (as above).
• May perform in-office procedures consistent with privileges and competency validation.
Documentation
• Outpatient visits are documented in EPIC system using SOAP format.
• Provides timely documentation in Epic; meets compliance standards; and captures patient
history, presenting problem, physical exam, diagnosis(es) and treatment/plan of care.

• Signs/closes visit notes within 24 hours of the patient encounter.
Other Responsibilities
• Reviews results of diagnostic studies in a timely manner, and communicates follow-up
plan to the patient
• Reviews and addresses telephone and Epic portal messages (In-Basket)
• Reviews specialist consults, recent inpatient summaries, or other patient information
copied from other providers
• Provides In-Basket coverage as needed when other providers are out-of-office
• On-call responsibility (including overnight availability via phone) shared with other
providers/team
Communication and Collaboration
• Maintains ongoing communication and collaboration with office staff and
interdisciplinary health care team.
• Facilitates and communicates with patients, patient care team, and staff
• Report compliance concerns or patient safety issues/events.
Quality and Productivity
• Engages with population health team to improve patient access and outcomes.
• Aims to meet annual quality metric goals as defined by Jefferson Medical Group.
• Meets target productivity, measured by work RVUs (relative volume units)
Professional Practice
• Demonstrates Professional Practice behaviors including: preceptor/mentoring, education,
and instruction of graduate students.
• Seeks opportunities for active engagement in research and the analysis of evidenced
based practice.
• Completes annual required health system learning activities
• Actively participates in Advance Practice Grand Rounds and specialty-specific meetings.
• Maintains national board certification, state licensure and required continuing education,

Qualifications
Master’s Degree
National Board Certification as a Primary/Family Care Nurse Practitioner or Physician Assistant
BLS certification
