Position Overview:
We are seeking a compassionate and dedicated Family Practice Nurse Practitioner (NP) to join our team at Friends of Family Health Center's Women's Clinics. The ideal candidate will provide high-quality, patient-centered care to individuals and families, ensuring access to comprehensive primary healthcare services in a culturally competent manner.

Responsibilities:
Conduct comprehensive health assessments, including physical examinations, medical histories, and diagnostic evaluations.
Develop and implement individualized care plans based on patient needs and evidence-based guidelines.
Provide preventive care, including immunizations, screenings, and health education.
Diagnose and treat acute and chronic illnesses, managing a diverse patient population.
Prescribe medications and therapeutic interventions in accordance with state regulations.
Collaborate with a multidisciplinary team to coordinate patient care and enhance health outcomes.
Educate patients and their families on health promotion, disease prevention, and management of chronic conditions.
Maintain accurate and up-to-date patient records in compliance with regulatory standards.
Participate in quality improvement initiatives and community outreach programs.
Stay current with medical advancements and best practices in family healthcare.

Qualifications:
Master’s degree in Nursing from an accredited program.
Current Nurse Practitioner certification with a focus on Family Practice.
Valid state Nurse Practitioner license.
Experience in a primary care setting, preferably in a community health or FQHC environment.
Strong clinical assessment and diagnostic skills.
Excellent communication and interpersonal skills, with a commitment to cultural competence.
Ability to work effectively in a team-oriented, collaborative environment.
Bilingual Spanish/English preferred.
Experience with underserved populations and community health initiatives.
Proficient in electronic health record (EHR) systems.

Working Hours

Full time, on-site Monday through Friday from 8AM to 5PM
