Benefits:

About the role:
- PRN - flexible, create you own schedule
- Create your own schedule, 20 assessments/month, 45-60 minute assessments
- Contact Center will build provider's schedule based on their availability
- APP will travel up to 120 miles per work day, mileage will be reimbursed
- Physician Assistant / Nurse Practitioner will work autonomously to see Medicare age patients in the home, performing annual comprehensive health assessments
Background:
- Board Certified Family Nurse Practitioner / FNP, Acute Care Nurse Practitioner / ACNP, or Adult-Geriatric Nurse Practitioner / AGNP
- Ideal background is a Nurse Practitioner with urgent care or ambulatory experience
- Active OH state license required
- Open to new grad Nurse Practitioners
