SCHEDULE:
- Full-Time and Part-Time Available
- 10-Hour Shifts
- 8 Hours Patient Care / 2 Hours Admin Time
- Full-Time is 40 Hours
- Part-Time Schedule Negotiable
- No On-Call
- No Nights
- No Weekends


FULL-TIME COMPENSATION:
- $105,000 to $130,000 Base Salary, depending on experience

COMPANY PROFILE:
This community health center (FQHC) opened in 2019 and currently employs 13 providers across 4 clinic locations and several neighborhood outreach centers. Their services include a full scope of primary care offerings, dental care, behavioral health, and more. They are currently hiring due to growth at their newest location in Montrose.


POSITION DESCRIPTION:
The new Nurse Practitioner or Physician Assistant (APP) will provide comprehensive medical services for patients of all ages. Shifts are scheduled in 10-hour blocks, with 8 hours designated for patient care and 2 hours dedicated for administrative time each day.

APPs see an average of 17 patients per day, with the support of dedicated clinical assistants - the clinic schedules 3 Medical Assistants per 2 providers.

Specific duties of the role include, but are not limited to:

- evaluation and treatment of patients
- care plan management
- assessment of acute needs
- chronic condition management
- review laboratory and other test results
- initiate specialist referrals
- participate in care team, provider, and staff meetings
- provide health education to patients and families

The office has behavioral health and dental providers on-site, with patient navigators to assist as needed with any needs that arise with the vulnerable patient population that they serve.

The practice currently uses AthenaHealth, but is in the process of transitioning to Epic.


REQUIREMENTS:
- Colorado NP or PA license
- DEA
- 2+ years advanced practice experience with all ages
