Requirements to apply:

• Medical Degree: A master’s degree from an accredited Nurse Practitioner Program
• Experience: A minimum of 2 years of experience
• Certification: A valid and current DEA Registration
• Preferences: Geriatric experience preferred
• Valid State Driver s License with a clean DMV record and copy of recent Vehicle Insurance required

We are seeking a skilled and dedicated Nurse Practitioner to join our Stockton and Modesto, CA Team!

Job Overview

Perform in-person comprehensive assessments including initial, semi-annual and unscheduled assessments and continuous medical coverage to participants. Collaborate with physicians and IDT to develop care plans for each participant. Collaborate with physicians and IDT in discharge planning from acute or long-term settings. Help maintain care of participants in the nursing home, making regular visits as dictated by participant needs. Work collaboratively with physicians and IDT to oversee and ensure effective care coordination. At the direction of the Medical Director, review outcomes and utilization data, including hospitalizations, and use of specialists to establish best practices and policy improvements. Take part in rotating night and weekend call.
