Schedule: 8:00 AM to 5:00 PM, Monday to Friday

Responsibilities as the Nurse Practitioner-Bilingual (Chinese):
- Care Coordination: Work closely with community physicians and the interdisciplinary team to create and implement tailored care plans for participants.
- Clinical Management: Conduct wellness assessments, manage acute conditions, and provide urgent care services.
- Supervision: Oversee nursing and clinical staff to ensure quality care and adherence to protocols.
- Documentation: Ensure accurate clinical records and proper documentation of all participant care.
- Education & Support: Provide education to participants, families, and staff on health management and self-care strategies.
- Education: Completed Nurse Practitioner program approved by NYSED.
- Experience: 2+ years in Clinical Nursing, with at least 1 year working with elderly or frail populations.
- Licensing: Licensed Nurse Practitioner in New York State; DEA Certification; AANP or ANCC Board Certification.
- Bilingual Requirement: Bilingual in Chinese and English required.
- Skills: Strong communication, analytical, and organizational skills, with proficiency in Microsoft Office.
