Details:
  • Setting: Provide Hospice/Palliative Care for Patients
    • Work autonomously in an APP role
    • Work collaboratively with the Palliative Care Medical Team to deliver appropriate care from initial assessment through discharge
      • Assessments include: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately-controlled pain or other distressing symptoms, falls, infections, impact of disease burden on patient, caregiver, and/or family's quality of life
    • Prescribe medications, order/interpret labs, order treatments or medical equipment
    • Have meetings with Palliative Medical Director as needed, but no less than biweekly
  • Mileage reimbursement provided
  • 4x10s - no weekend schedule
Required:
  • Minimum of 2 years of NP experience required
  • ACHPN certification preferred
  • Active BLS and DEA required
Compensation:
  • Receive a paycheck every Friday
