Location: Hartford, CT
Salary: $130,000 - $165,000 + Bonus + Full Benefits + Paid Relocation

Department: Adult Medicine / Primary Care

We are seeking a seasoned Nurse Practitioner (APRN/NP) or Physician Assistant (PA) to join our Adult Medicine clinic in Hartford, CT. This role involves providing comprehensive primary health care, including health promotion and disease prevention for adult patients. The ideal candidate will work collaboratively with department physicians and refer patients to specialists as needed for diagnosis confirmation and treatment procedures.

Key Responsibilities:

  • Deliver high-quality primary care to adult patients, including health promotion and disease prevention.
  • Collaborate with physicians to provide specialized care and make appropriate referrals.
  • Take comprehensive medical histories, assess medical conditions, and interpret diagnostic findings.
  • Ensure all clinical documentation is accurate, thorough, and completed in a timely manner.
  • Utilize evidence-based practices to make clinical decisions and adhere to department quality measures.
  • Provide culturally competent care to a diverse patient population.
  • Communicate effectively with patients, colleagues, and other stakeholders both orally and in writing.
  • Use electronic health records and other computer systems proficiently.
  • Perform additional duties as assigned.

Qualifications:

  • Graduation from an accredited school of nursing with successful completion of an approved nurse practitioner program, or completion of an accredited PA program.
  • Valid State of Connecticut licensure.
  • Eligibility to obtain DEA and state narcotic registration.
  • Strong foundation in nursing or physician assistant theory and practice.
  • Proven track record of delivering high-quality clinical care in accordance with established standards.
  • Ability to provide culturally competent care in a collaborative environment.
  • Excellent communication skills, both written and oral.
  • 1-2 years of experience in an ambulatory setting or urgent care preferred.
  • Must pass a pre-employment background check and drug screen.
