Job Responsibilities

Variable remote and onsite schedule based on the needs of the clinics.
Minimum requirement of 2 weeks per month on site at a clinic.
Office hours are Mon - Fri 8:00am to 4:30pm.
This is a Full-Time Float position, and we are looking to hire 2 individuals.
Examine the patient/client and determine the best course of action while documenting in our paperless health program, VISTA/CPRS electronic patient record system.
Develop and implement patient management plans, communicating with Patient Aligned Care Team (PACT) to ensure appropriate standards of care.
Discuss patient outcomes, policies, procedures, and records with Medical Director.
Completes all clinical reminders to keep each patient up to date in the system.
This job requires access to confidential and sensitive information requiring ongoing discretion and secure information management.

Qualifications

Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioners (MSN required).
Board-certified in family practice or internal medicine
Certification by an applicable professional organization
1 year of clinical medical practice preferred
Experience with VISTA/CPRS electronic patient record system is required
Current, unrestricted license in the state of desired employment
Licensed in AZ, NM, TX, USVI within the first 6 months of hire date
Current BLS for providers from the American Heart Association (to be renewed annually)
Current, unrestricted Drug Enforcement Administration (DEA) registration
Excellent computer skills
