The Primary Care Provider treats patients and provides quality health care through direct contact with patients in the clinic and via mediated communication, such as telephone calls, video chatting, or online chatting. Regional Directors, Clinic Directors and Supervising Physicians will be an area of contact for support. The Primary Care Provider will interact closely with all members of the clinics, including but not limited to, telehealth team members, other Primary Care Providers, and telehealth call center team member, to achieve excellent patient care. Additionally, the primary care provider will assist and support other care referrals to support the patient’s overall health. WHAT YOU WILL DO: - Conduct thorough physical examinations and assessments for patients of all ages, including children ages 3 years and up. - Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma. - Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings. - Order, interpret, and evaluate diagnostic tests to diagnose and assess patients' clinical problems and healthcare needs. - Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans. - Prescribe medications, recommend treatments, and provide written home-going instructions. - Refer patients to specialists or other healthcare resources as needed. - Educate patients and their families on health maintenance and preventive care. - Promote patient engagement, health literacy, and shared decision-making. - Participate in quality improvement initiatives and value-based care programs. - Emphasize patient-centered care, continuity of care, and building strong patient-provider relationships.
