We seek an experienced FNP to join our primary care team, dedicated to providing value-based care through a virtual telehealth platform. This innovative role emphasizes a team-based approach, collaborating closely with our population health team, practice manager, and support staff. The successful candidate will play a crucial role in closing care gaps and ensuring our network members receive comprehensive, high-quality care.

Key Responsibilities:

• Provide primary care services via telehealth to patients within Banner plans and network
• Collaborate with established primary care providers to ensure continuity of care.
• Work with the population health team to address care gaps and improve patient outcomes
• Focus on value-based care quality measures and population health initiatives
• See an average of 16-18 patients per day

Qualifications:

• Must live in the Phoenix valley for occasional onsite functions
• FNP designation with AZ license eligibility
• Minimum 2 years of clinical experience as an FNP in a primary care setting
• DEA with prescriptive authority required
• Passion for value-based care and population health
• Comfortable with telehealth and virtual patient interactions
• Patient-focused, friendly, and willing to collaborate in a team environment
