Job Description:
At Family Medicine Associates we provide patients the best care by offering full spectrum Family Medicine services including behavioral health integration for individuals and families. Our Thornton office is located 15 minutes north of Coors Field, and a short 20-minute drive from Boulder, CO. Close to great biking, hiking and various entertainment venues.

The Position:
Performs a comprehensive patient health assessment, including health history, physical exams and preventive screening.
Sees 12-17 patients a day
Orders, administers and analyzes diagnostic tests.
Procedures- Female health, biopsies, blood tests
Establishes systematic follow-up for evaluation of the plan of care and reassesses and modifies the plan, as necessary, including initiation of referrals and consultation with physicians and other health care providers as appropriate.
Maintains accurate, legible and confidential records.
Trains patients and families on managing illnesses or injuries and disease prevention.
Develops and maintains a good working relationship with the healthcare team.
Qualifications (Education/Experience/Knowledge/Skills/Abilities):
Possess current, valid licensure as an Advanced Practice Nurse from the Colorado Board of Nursing and DEA license

Job Schedule: Up to 4 days a week
Shift: Days
Workdays: Example: 4 days per week; usually Monday through Thursday

Salary Range:
- Base salary $72,800 - $117,000
