Requirements to apply:

• Experience as an RN for 2 years with complex populations, prior to becoming an NP.

• Active, unrestricted NP license and CPR certification

• If NP, RN California State License

• Master’s degree and graduate of accredited NP/PA program

• Graduation from an Accredited School of Nursing

• Certificate as a Nurse Practitioner with Prescriptive Authority

• Current knowledge of chronic care/geriatric issues and best practices

• Proof of a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law.

Job Overview

In this role, you will serve on an interdisciplinary team (IDT) and provide direct medical care to participants. You will work collaboratively with the IDT to create and drive forward a care plan that improves health outcomes for your panel of participants. In this role, you will meet participants where they are – whether that be in the center, clinic, home, virtually or an external site of care.
