Job description

  • Works as part of a team, using a participatory style of service delivery, to provide quality nursing care to all EHFM clinic patients.
  • Documents and maintains health care records in EHFM’s electronic medical record system.
  • Initiates a comprehensive, ongoing identification of patient needs in order to develop and implement a plan of services and advocacy for that patient under the direction of the Clinic Director.
  • Follows case management plans and procedures per EHFM’s Administrative and Clinical policy and procedures manual.
  • Serves as the primary provider. Provides patient diagnosis, treatment, consultation, and follow-up under the supervision of one or more physicians.
  • Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care.
  • Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral.
  • Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues.
  • Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications.
  • Maintains legible, accurate, and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations consistent with EHFM procedures and state regulatory standards.
  • Facilitates evaluation of records by physician(s), peers, and Quality and Standards according to protocols, and receives and implements constructive directives.
  • Utilizes new knowledge gained from conferences, workshops, professional literature, or "hands-on training" and assimilates this knowledge into clinical practice.
  • Obtains complete and accurate medical histories and provides patient education.

Supplemental schedule:

  • On call - Rotates on a weekly basis

Weekly schedule:

  • Ideally Monday- Friday 8 a.m. to 4:30 p.m but there is flexibility (30 hours and up is considered full-time)
  • Four 10 hour shifts / week or five 8 hour shifts per week
  • No weekends

Experience:

  • 3 years experience as a Nurse required
  • Willing to accept new grad or Board Eligible Nurse Practitioner
