Comprehensive Primary Care (CPC) is seeking compassionate and dedicated Family Nurse Practitioners to join our dynamic team in Maryland. The successful candidate will play a crucial role in screening and managing community patients for prevalent diseases, addressing their health concerns, and performing family medicine provider duties. Job Responsibilities
Diagnose and manage acute and chronic illnesses, with a focus on preventive care and health maintenance.
Develop and implement individualized treatment plans in collaboration with patients and their families.
Provide education and counseling on health promotion, disease prevention, and management of medical conditions.
Collaborate with our board-certified physicians and specialists for consultation and referral when necessary.
Maintain accurate and up-to-date medical records and documentation.
Participate in quality improvement initiatives to enhance patient care outcomes.
Qualifications
Active and unrestricted state nurse practitioner license.
Strong clinical skills and the ability to work independently.
Excellent communication and interpersonal skills.
Commitment to providing high-quality patient care and promoting a positive patient experience.
Experience with EHR strongly preferred.
Spanish bi-lingual skills a plus.
