Job Description

An East LA, California based client is seeking an exceptional Nurse Practitioner to provide compassionate primary patient care. An ideal candidate is motivated, collaborative, and works well in an interdisciplinary team. This clinic is a fast-paced environment and providers are responsible for medical services to all ages. This role involves in home and telehealth Annual Wellness Visits.

Required Qualifications

• CA medical license as NP
• CA DEA certificate
• Current NP board certification by ANCC/AANP
• CPR certification

Responsibilities

• Assess and diagnose a variety of diseases and injuries in patients of all ages
• Perform therapeutic procedures, including preventative care and follow up
• Conduct Annual Wellness Visits for patients in home and through telehealth services
• Arrange referrals, consultations, and therapeutic services
• Chart review and document all patient encounters efficiently and according to organization policy and JCAHO standards
