Position: Advanced Practitioner - Family Nurse Practitioner (FNP) or Physician Assistant (PA)
Location: Hospital-Based Practice (Days, Monday - Friday, No Call)
Job Summary
We are seeking a dedicated and skilled Advanced Practitioner, either a Family Nurse Practitioner (FNP) or Physician Assistant (PA), to join our hospital-based practice. This position offers a Monday to Friday schedule with no call responsibilities, providing an excellent work-life balance. As an Advanced Practitioner, you will play a crucial role in delivering comprehensive healthcare services to our patients.
Responsibilities
- Patient Assessment: Conduct thorough assessments of patients, including medical history, physical examinations, and diagnostic tests to formulate accurate diagnoses.
- Treatment Planning: Develop and implement individualized treatment plans, including medication management, preventive care, and health education, in collaboration with the healthcare team.
- Chronic Disease Management: Manage chronic conditions by monitoring patients' progress, adjusting treatment plans, and promoting lifestyle modifications.
- Acute Care: Provide immediate care for patients with acute illnesses or injuries, ensuring timely interventions and referrals when necessary.
- Patient Education: Educate patients and their families on healthcare maintenance, disease prevention, and self-care strategies.
- Collaborative Care: Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.
- Documentation: Maintain accurate and up-to-date electronic health records (EHR) for all patient encounters.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care outcomes.
- Compliance: Adhere to all regulatory and legal requirements, including licensure, certification, and hospital policies.
- Professional Development: Stay current with advancements in healthcare through continuing education and professional development.
Qualifications
- For FNP: Current licensure as a Family Nurse Practitioner (FNP) in New York with prescriptive authority.
- For PA: Current licensure as a Physician Assistant (PA) in New York.
- For Both: Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) for FNPs; Physician Assistant (PA-C) certification for PAs.
- CPR certification and ACLS/BLS certification preferred.
- Strong clinical and diagnostic skills.
- Excellent communication and interpersonal abilities.
