Job Description
LTC Physician Care, PC provides Primary Care (PCP) services to Skilled Nursing and Long-Term Care facilities across Michigan. Our goal is to improve with compassion the overall quality of care, patient outcomes, quality of life, refine facility processes, and enhance patient relationships.
LTC is seeking a dedicated Nurse Practitioner to join our team as Primary-Care Providers.
Requirements and Qualifications
- Experience in Family Medicine, Internal Medicine, or Geriatrics preferred.
- We are happy to consider new graduates.
- Candidates must hold a valid and unrestricted medical license in Michigan.
- Candidates must hold a valid DEA license in the state of Michigan.
- Candidates must have completed a valid U.S. certification program
Job Structure
- This is an in-person position
- Providers will commute directly between home/facilities
- Nurse Practitioners work in coordination with an LTC Collaborating Physician
- Nurse Practitioner is responsible for the day-to-day care of their patients
- After-Hours on-call is shared among all LTC Providers
Compensation and Benefits
- Productivity Based Compensation Model (No Cap)
- $10k SIGN ON BONUS
- 15 PTO days annually
- $2,500 Annual CME allowance plus 3 PTO days to use for CME attendance

- Monthly phone stipend
