We are seeking an experienced Nurse Practitioner or Physician Assistant with a strong clinical Dermatology background to join our team in Texas. You will help provide in-person services to our post-acute patients in facilities in the Dallas, Houston or San Antonio areas and provide follow-up telehealth visits, as needed. Under the guidance of a supervising Dermatologist, and as part of an interdisciplinary care team, this position will provide in-person and telehealth visits as part of ongoing medical management and acute interventions for patients in the post-acute care setting. You will play an essential role in enhancing the well-being and assisting each patient return to their highest level of independence. DUTIES AND RESPONSIBILITIES, may include but not be limited to
- Assess, diagnose, and manage a diverse range of dermatological conditions/wounds.
- Coordinate the patient plan of care with the physician and the facility's clinical disciplines to address patient needs, evaluate the effectiveness of care, and assess changes to the patient medical condition to ensure outcome goals
- Establish plan of care and work within protocols established by the supervising physician
ESSENTIAL ABILITIES
- Ability to provide both telehealth and onsite visits to patients in skilled nursing facilities near your location in the Dallas, Houston or San Antonio areas.(75% telehealth and 25% in-person)
- Due to the hybrid nature of this job, applicants living in the Dallas, Houston or San Antonio areas will have priority.
- Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday, 2 or more hours time block each time
- Excellent communication skills and ability to effectively communicate with patients, family members, and other medical professionals.
QUALIFICATIONS
- Master's degree in Nursing Licensure, Certification, and Registration
- Current, valid Texas APRN license Certification through the ANCC, AANP (or equivalent certification body)
- Current BLS required
- Medicare NPI #, 855I and 855r on file (Independent NP Medicare Provider Number/PTAN#)
- Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday.(in 2 or more hours time block each time)
- No weekends or evenings at this time.
- Dermatology-focused clinical experience (3+ years required).
- Geriatrics and telehealth experience are highly preferred.

Our clinician-focused technology and resources will help you:
- Conduct clinical assessments and evaluations.
- Formulate treatment plans as needed.
- Prescribe medications with proper dosage and administration instructions.
- Oversee telemedicine patient flow and daily functions of the telemedicine shift.
- Maintain accurate electronic medical records of patients' contact details, medical history, prescribed medications, allergies, diagnoses, and progress.
- Maintain integrity, availability, and confidentiality of all PHI
- Oversee the reporting of diagnostic testing results, follow-up treatment, referrals, and recovery.
- Participate in quality assurance and care improvement initiatives.
- Adhere to HIPAA standards for information security, privacy, and confidentiality.
