A leading healthcare organization is seeking a dedicated and compassionate Advanced Practice Provider (FNP or PA) to join their primary care team. Based in a juvenile detention facility, this role involves delivering high-quality healthcare to adolescent boys.

Responsibilities:

  • Conduct comprehensive assessments, including detailed physical examinations and evaluations of psychosocial needs.
  • Diagnose, treat, and manage acute and chronic conditions commonly seen in primary care.
  • Provide patient education and counseling to promote self-management and improve health outcomes.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Ensure timely access to preventive care and screenings in accordance with healthcare guidelines.
  • Serve as a clinical resource to patients, families, and staff members within the facility.

Qualifications:

  • Active FNP or PA license in the state of New York.
  • Willingness to work with adolescent boys in a juvenile detention setting.

Desired Skills:

  • Experience in urgent care, primary care, or corrections settings (new graduates welcome to apply).
  • Strong interpersonal and communication skills, with the ability to build trust and rapport with patients.
  • Familiarity with managed care and preventive health approaches is a plus.

Compensation & Benefits:

  • $125,000-130,000 base salary.
