We seek a Nurse Practitioner or Physician Assistant with strong cardiology experience to provide telehealth visits with seniors in post-acute facilities in Texas, focused on screening, diagnosing, and formulating care plans specific to each patient's health goals. Under the guidance of a supervising physician, this Nurse Practitioner will assist with assessment, diagnosis, and treatment of cardiovascular disease. Duties and responsibilities include but are not limited to, performing comprehensive medical assessments, ordering appropriate tests/procedures for diagnostic purposes, establishing treatment plans, and obtaining specialists' consultations, as needed.
CARDIOVASCULAR
This role encompasses patient evaluations relating to cardiovascular symptoms or known cardiovascular illnesses, including congestive heart failure, coronary artery disease, hypertension, and atrial fibrillation. You will use your expertise to assist with educating and supporting patients and their families, choosing appropriate diagnostic testing, and establishing interventions and treatment plans using evidence-based medicine.
ESSENTIAL ABILITIES
- Ability to provide telehealth visits to skilled nursing facilities.
- Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday, 2 or more hours' time block each time
- Excellent communication skills and ability to effectively communicate with patients, family members, and other medical professionals.
QUALIFICATIONS
- Master's degree in Nursing Licensure, Certification, and Registration
- Current, unrestricted Texas APRN license Certification through the ANCC, AANP (or equivalent certification body)
- Current BLS required Medicare NPI #, 855I and 855r on file (Independent NP Medicare Provider Number/PTAN#)
- Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday.(in 2 or more hours time block each time)
- No weekends or evenings at this time.
- Cardiology-focused clinical experience (3+ years)
- Geriatrics and telehealth experience are highly preferred.
