PA heart and Vascular group at Jefferson Health located in Jenkintown, PA is seeking a full-time Nurse practitioner to work solely in the outpatient office setting. The CRNP will work with a seasoned group of cardiologist and cardiology CRNPs performing clinical activities related to assessments and evaluations as well as diagnostic and therapeutic tasks for patients ranging in ages from 18 years to 100 years, involvingpre, peri, and post-partum patients and oncology patients.

The Advanced Practice Nurse (CRNP) within the ambulatory setting works in collaboration with, and is an integral member of a multidisciplinary health care team. The APP provides comprehensive care in the cardiology ambulatory practice.

  • M-F, full-time ambulatory (1.0 FTE)
  • Practice office hours range from 8:00am -4:30pm
  • No in-patient responsibility
  • No on-call responsibility
  • No weekend responsibility
  • Assist with patient communication and provide coverage for colleague providers as necessary

Scope of Care to be Provided:

  • Perform complete history and physical examinations, formulate differential diagnosis, propose plan of care and document said findings
  • Order and collect data using established guideline-based assessment techniques
  • Manage diagnostic procedures and therapeutic interventions through ordering, interpretation, performance and supervision
  • Assess for risks associated with the care of the acute and complex cardiac patient
  • Identify complications and orders appropriate interventions
  • Review and interpret diagnostic tests and laboratory data and present/review findings of concern with the colleague physician
  • Understand and apply evidenced based practice guidelines and institution/program specific protocols to help create individualized plans of care
  • Perform advanced procedures consistent with privileges and competency validation

Cardiology Specific Responsibilities:

  • Educate patients with cardiac risk factors, acute and chronic heart disease, congestive heart failure and pacer/defibrillators
  • Perform pharmacologic and exercise nuclear stress testing and stress echocardiograms as needed

Documentation:

  • Documents in the medical record key components of patient’s progress
  • Documentation shall be timely and meets compliance standards and captures patient acuity

Communication and Collaboration:

  • Maintain ongoing communication and collaboration with interdisciplinary health care team
  • Facilitates and communicates with patients, family and staff to promote continuity of care across the outpatient and inpatient continuum of care and ensure patient safety

Professional Practice:

  • Demonstrate Professional Practice behaviors including but not limited to preceptor/mentoring, education and instruction of graduate students
  • Seeks opportunities for active engagement in research and the analysis of evidenced based practice
  • Actively participate in Advanced Practice meetings
  • Maintain CEU’s and membership in a professional organization
  • Master’s Degree
  • National Board Certification as a Primary/Family Care Nurse Practitioner/Licensed by PA State Board of Nursing
  • BLS and ACLS certification
  • One to two years’ experience in primary care with cardiology experience preferred
  • EKG interpretation preferred
