Job Summary

Acute Hospital Care at Home (AHCaH) providers operate similarly to inpatient hospitalists with a more virtual care approach. Cooperating with other team members through huddles and as needed to assess patient needs and identify appropriate patients for this model of care.

Job Duties

Through reliable communication, remote patient monitoring and outlined protocols, AHCaH enables patients who meet hospital admission criteria to receive care in the comfort of their home.

Target Population:

  • Med/Surgical level of care

    • Congestive Heart Failure/COPD exacerbation
    • Urinary Tract Infection
    • Potential for other appropriate conditions

Primary health team includes:

  • Advanced Practitioner (AP)- Core provider, works closely with patients and care team under physician guidance.
  • RN Coordinator
  • Field Staff (RNs and Mobile Paramedics
  • Physician, direct care and interdisciplinary team supervision.

Location: Geisinger Health Plan Hughes Center, Danvil

  • with travel as needed to Geisinger Medical Center, Geisinger Bloomsburg Hospital and Geisinger Shamokin Community Hospital

Schedule: 7 days on; 7 days off

Hours: 7 a – 7 p

Call schedule: rotates with physicians and other Advanced Practitioners
