Requirements:

• Board Certified CRNA – REQUIRED
• Active MI license – REQUIRED
• Clean Background- REQUIRED
• BLS ACLS – REQUIRED
• Conversion Rate cap- $18k- Required at time of presentation
• Must meet rate cap of $250 inclusive of malpractice and travel
• Local Providers- Highly preferred- will consider travel providers if travel is included in rate cap of $250
• Looking for a provider that is willing to do pediatric cases- Required
