Requirements:

• NBCRNA Certified – REQUIRED
• Active CO License – REQUIRED or IMLC (LOQ IN HAND AT TIME OF SUBMISSION)
• Active BLS – REQUIRED
• Must disclose the # of years of Epic Experience and the Last MM/YYYY provider used EPIC – REQUIRED
• Clean Malpractice/Background – REQUIRED
• COVID VACCINATED – REQUIRED
• Must meet rate cap of $255 per hour (inclusive of all travel)
