Requirements:

• Board Certified CRNA – REQUIRED
• Active MI license – REQUIRED
• Clean Background- REQUIRED
• BLS ACLS PALS – REQUIRED
• Conversion Rate cap- $18k- Required at time of presentation
• Must meet rate cap of $250 – inclusive of malpractice- Required at time of presentation
• Any time off needs to be requested up front- Required
• Full time required
• Clean background- hard requirement for quick turnaround
