Requirements:

• NBCRNA – REQUIRED
• Active MI license or IMLC – REQUIRED
• Clean Background – REQUIRED
• BLS ACLS – REQUIRED
• Required to work a minimum of 3 days per week – Required
• Conversion Rate cap – $18k – Required at time of presentation
• Must meet rate cap of $250 inclusive of malpractice and travel
• Local Providers – Highly preferred – will consider travel providers if travel is included in rate cap of $250
