Car Rental/Mileage Stipend- $250/week

Lodging/Housing Stipend- $147/Daily

Must work 5 8's CAA's Welcome and expectations are 5(8)'s or 4(8)’s

What is the daily shift time? 8 hours, 7:00-3:30pm with a 30 min lunch

What cases would locum need to cover: OB? NO Ortho Cases? Yes Neuro/Heads? Yes Pediatrics? NO

Is there CRNA Supervision or docs doing own cases? 4:1, Care Team Model a. If supervision, provide ratio.

Would there be any OT to be expected? No

Is there call involved? No

How many anesthesia rooms are on site? 25-27 rooms

Type of charting used. If electronic, give name system. Meditech Paper

Privileging time frame? Less than 30 days (emergency credentialing)

How many providers needed? 2
