Requirements:

• Active MI license
• Submit rates at the time of name clear
• Covid vaccinated
• BLS/ACLS certified
• Board Certified
• Notate participation in MI Medicaid/CHAMPS at time of name clear
• Clean background with no disciplinary actions and no malpractice
• DEA – Verbal Confirmation Required at Name Clear
