I: Qualifications:

Minimum of a master’s degree from an accredited college or university in the field of Human services, with at least 1-years direct work experience in Mental Health working with individuals with a mental health diagnosis. Must be a Licensed Mental Health Professional or Licensed Professional Counselor (LMSW, LPC-A) in good standing with the Texas Board of Health Professionals; preferably 2 years of experience in the direct supervision of QMHPs and experience in diagnosing, treatment, and direct service delivery to individuals with serious mental illness and serious emotional disturbance. Must have experience in managing and development of program. The employee must have successfully completed a criminal history, central registry background investigation, and present an acceptable driving record.

II: Essential Job Functions:
  1. Provide clinical supervision on documentation and support to the QMHP’s on a weekly/ daily basis.
  2. Review, complete and sign Comprehensive Assessments, individualized Crisis Education and Prevention Plan (CEPP), Individual Service Plans (ISPs),
  3. Service Registration Forms, Therapy Session/Daily Supervision, and Continued Stay Service Authorization Requests.
  4. Attend all scheduled trainings for CMBHS, TMHP and effectively implement and communicate if any, the changes to assure compliance with licensure no requirements. Provide leadership, guidance, and clinical direction to staff, in developing and evaluating effective programs, responsive to the need of identified target population.
  5. Coordinate with other agencies and community resources to enhance services through interagency collaboration.
  6. Make presentation to referral agencies about Freedom Medical Center, LLC, and services. Serve as a clinical member of the management team.
  7. Ensure agency compliance with all relevant laws, licensure regulations, and program policies.
  8. Assess the mental and behavioral health status of referred patients.
  9. Formulates differential diagnosis of mild to moderate mental health conditions and/or substance abuse.
  10. Formulates short-term and long-term counseling plans for patients under care.
  11. Identify patients with urgent mental health conditions (e.g., suicidal, homicidal, psychotic, etc.) and make appropriate referrals to available emergency or psychiatric treatment facilities.
  12. Provide written documentation of counseling sessions, assessments, interventions, and recommendations for FMC providers.
  13. Provides, individual, group and family therapy session as indicated.
  14. Provides in-service training on relevant topics for co-workers as requested.
  15. Participate in a team management approach to patient care/treatment.
  16. Participate in a team management to provide reports for quality improvement plan.
  17. Orient individual’s’ rights as it relates to human rights regulations.

III: Knowledge and Skills:
  1. Knowledge of the principles, techniques, and practices related to the provision for community
  2. based mental health support services
  3. Knowledge of program evaluation activities and casework documentation and requirements to
  4. assure compliance with regulatory requirements.
  5. Extensive knowledge of mental illness and mental health diagnoses and assessment.
  6. Considerable knowledge of principles and methods of clinical supervision.
  7. Knowledge of community resources and entitlement benefits
  8. Possess a certification in First Aid, CPR, Behavior Intervention
  9. Texas Driver’s Record.
  10. Possess vehicle insurance
  11. Possess access to a computer and internet
  12. Must complete a clear/eligible field print criminal background check, and CPS check.


Informing prospective clients about pertinent financial and emotional commitments:
  1. Consulting with new clients to determine their difficulties and resources.
  2. Devising tailor-made treatment regimens and adapting these, as needed.
  3. Addressing concerns through talk therapy and by modeling adaptive, sustainable behaviors.
  4. Promoting constructive self-reflection, helpful coping techniques, and nurturing relationships.
  5. Monitoring perceptions about and adherence to suggested treatment.
  6. Recording symptomology over time to ascertain the utility of interventions.
  7. Writing clinical and suitable referral notes, as required.
  8. Staying warm, engaged, and receptive as you work to facilitate healing.
