Join our UPMC Post-Acute Care network providing primary care to older adults who have complex health care needs as well as consultations for patients with specific geriatric conditions. Excellent opportunity for rapid practice growth and development.
Nurse practitioner will provide quality patient care in various locations such as a skilled nursing or assisted living facility, our Carlisle Rehab, and patient's residence within four (4) counties (New Cumberland, Dauphin, Perry, York)
About the position:
We offer a patient-centered medical home model
Transition to care visits from hospital
Position allows for flexible work schedule within traditional office hours
Very light call shared amongst provider team
Innovative care model, optimizing provider-patient interaction
UPMC Central Pa utilizes a system wide Epic EMR
12 week orientation program
Transition to care visits
Provide procedures – not limited to – Adjust medications, check blood pressure/vitals, take and draw labs (blood/urine analysis), change wound bandages
Desired Candidate:
Graduate of accredited CRNP program
Adult-Gero or Family Nurse Practitioner preferred
Three (3) Experience
Commitment to providing high-quality, compassionate care to patients
Team oriented with ability to provide personalized patient care and demonstrate clinical outcomes
Eligible for licensure in the state of Pennsylvania
